General Information for All Clients
This notice covers essential topics, including risks and benefits of therapy, payment policies, and additional procedures. It is important for all clients to understand these details. The full document is available as part of the intake process, and you can request a copy at any time.
In Case of Crisis
If you are experiencing a mental health crisis or feel unsafe, or believe you may harm yourself or someone else, contact your local crisis unit or visit the nearest emergency room. Your safety and well-being are our top priority.
Limits of Confidentiality
Your counselor will not share your Protected Health Information (PHI) without your consent, except in the following circumstances:
Risk of harm to yourself or others
Suspected abuse (child, elder, vulnerable adults, or animal abuse)
Court-ordered disclosures
Payment processing
Any disclosure will be limited to only the relevant information, adhering to confidentiality regulations.
Authorization for Information Sharing
If you want your treatment information shared with others (family members, legal counsel, healthcare providers), please complete a Release of Information (ROI) form.
Consultation with Colleagues
Your counselor may consult with other professionals to improve your care. This will involve sharing minimal information to ensure the best treatment plan.
Unplanned Absences and Professional Will
In case of an unexpected absence, a Professional Will will ensure that an Executor manages your care and informs you of any changes.
Risks and Benefits of Therapy
Therapy can provide emotional resilience, personal growth, and better self-awareness but may also involve challenges like initial discomfort, emotional distress, or changes in relationships. Understanding these risks is crucial for engaging fully in therapy.
Payment Policies
Please be aware of all costs, including documentation fees, court appearances, and the fee schedule for services. Payment is expected at the time of the request, and we accept various payment methods. If you have an outstanding balance, services may be suspended until it's cleared.
Appointments and Waitlist Policies
You will receive reminders about scheduled sessions. If you need to cancel or reschedule, please do so as soon as possible to avoid a no-show fee.
Technology Requirements
To participate in telehealth services with Dryad Counseling, you must have:
A device capable of video conferencing (smartphone, tablet, or laptop)
A valid email address and phone number
A quiet, private, and comfortable space for your session
If you experience technical difficulties, your counselor will attempt to continue your session by phone if possible. For initial sessions, video is required by most insurance plans. Please ensure that you have a reliable internet connection to avoid session disruptions.
Client Rights
You have the right to be treated with dignity, to privacy, to refuse services, and to be informed about your treatment and any changes to your care plan. You also have the right to file a grievance if you feel your rights have been violated.
